Sole Account Opening Form
Instructions:
- This form will take 15 minutes to complete if you have all the information available. All fields are mandatory. Indicate N/A where it is not applicable.
- For Payroll, you will be provided a sole savings account. Should you prefer to open a peso checking account for your payroll, please indicate this in New Transactional Account Details.
- By default, you will receive your statements via e-mail. You will also be able to view your statements by enrolling to online banking. For delivery of printed statements, delivery fees may apply.
- In order to serve you better, you will be offered Phone Banking and online banking services and be issued a security token upon account opening. To opt out of Phone Banking, online banking, or request for a new Phone Banking PIN, please contact our Phone Banking hotline 8858-0000 (Personal Banking) or 8858-0800 (Premier clients).
- For Child Accounts, a Personal Details Form (PDF) must be accomplished by parent/guardian. Only a sole savings account can be opened for Child Accounts.
- For students, you will be required to present your valid school ID and photo-bearing government-issued ID(s).
- For joint account applications, Applicant 1 will be the main account holder and the correspondence will be sent to the Applicant 1 mailing address.
